FREE Athletic Physical Information
Licking Memorial Health Systems (LMHS) is happy to once again offer the Sports Screening Program for the Licking County Community in 2023. The program will offer screenings to all student athletes and band members, entering grades 7 through 12, required to comply with the Ohio High School Athletic Association sports physical pre-participation requirements. Students entering grades 11 and 12 in specific educational training programs at CTEC will also be eligible.
The program will include sports physicals, heart screenings, and baseline concussion testing. Heart screenings are provided to student grade levels every other year, according to LMHS program guidelines, and performed on students going into grades 7, 9, and 11, as part of medical clearance for the pre-participation physical. ImPACT baseline concussion screenings are also provided in the grade levels every other year (grades 7, 9, and 11) and an option available for athletes that play contact sports and do not have concussion screening available at their high school. Students who may have already completed a physical through their school are still able to register for just the heart screenings and/or ImPACT testing, if it is not available through the school and they would like to have these screenings done.
Registration for the program is open to the community on the LMHS website beginning April 1, 2023. Parents/students can select a date from the calendar of available sessions to sign up for the day that will work for their individual schedules. The program is free to all participants; however, registration is required to participate. The program dates will begin May 2nd and will end May 31st, with the first two dates located at the Pataskala Health Campus. Each scheduled night will be open to the first 90 individuals to register. Attached is a flyer for you to post at your school or handout to your student athletes with the details for registration. Once an individual registers for the program they will be linked to the packet of LMHS forms required to be complete prior to their appointment and the OHSAA pre-participation physical form. If your school utilizes a physical form other than what is provided by OHSAA, please have your athletes complete that form in advance to bring with them the night of their appointment; otherwise, they will need to complete the OSHAA form prior to check in. Please let me know if you have any questions.
Thank you in advance for your support of the LMHS Sports Screening Program!
